Re-print Lost/Stolen Cards or Certificates​

If you need a re-print of your statement of attainment, or ID Card please complete the request below and submit. Our administration staff will contact you to make payment and then will issue you a new certificate or ID Card.

Please Note:  You can get a transcript of your certificate by logging into your USI account.

VOC/SOA Card
$33+2.2% debit/credit card fee + postage = $35.16

Statement of Attainment
$33+2.2% debit/credit card fee + postage = $35.93

Statement of Attainment & Plastic Card
$55+2.2% debit/credit card fee + postage = $58.41
